District Policies and Requirements

  • Medication in Schools
    Before any medication may be administered to or by any student during school hours, the Board requires a written request by the parent and a written order by the prescribing physician. Specific forms are available through the nurse's office at each school regarding the administration of medication in school, as well as student self-direction and administration of medication.

    Immunization Requirements 
    New York State law requires written proof of the following immunizations:
    Kindergarten through Grade 1:
      4-5 doses DTP/aP, 3-4 doses Polio, 2 doses MMR, 3 doses Hepatitis B, 2 doses of Varicella.  
    For Grades 2-5: 
     4-5 doses DTP/aP, 3 doses Polio, 2 doses MMR, 3 doses of Hepatitis B, 1 dose of Varicella.
    For Grades 6 & 7:  3 doses DTP/aP, 1 dose Tdap, 3-4 doses Polio, 2 doses MMR, 3 doses Hepatitis B, 2 doses of Varicella.  7th Grade ONLY requires 1 dose Menactra or Menveo. 
    For Grades 8-12:
     3 doses DTP/aP, 1 dose of Tdap, 3 doses Polio, 2 doses MMR, 3 doses Hepatitis B, 1 dose Varicella.   Grade 12 ONLY requires 1 booster Menactra or Menveo.  Students transferring from a parochial school to a public school are considered new entrants.  Acceptable proof of immunizations must be from a doctor or NYSIIS.  Parental history is not acceptable.

    Dental Health Certificate
    New York State law (Chapter 281) permits schools to request a dental examination in the following grades: school entry, K, 2, 4, 7 & 10. Your child may have a dental check-up during this school year to assess his/her fitness to attend school. Please take the Dental Exam Form to your dentist and return the completed form to the school’s nurse as soon as possible.

    Concussion Management
    As per the New York State Education Department, any student who exhibits signs, symptoms or behaviors consistent with a concussion (such as loss of consciousness, headaches, dizziness, confusion or balance problems) shall not return to Physical Education or Athletics until cleared by the school medical officer (School District Physician).
